This Privacy Policy governs the manner in which QCD of America (“QCD of America “) collects, uses, protects and shares information collected from users (each, a “User”) of the www.qcdofamerica.com website (“Site”). This privacy policy applies to the Site and all products and services offered by QCD of America.
We may collect personal identification information from Users in a variety of ways, including, but not limited to, when Users visit our site, register on the site, place an order, subscribe to a newsletter, respond to a survey, fill out a form, and engage in other activities, services, features or resources we make available on our Site. Users may be asked for, as appropriate, name, email address, mailing address, phone number, date of birth, dependent information, or credit card or other billing information. The QCD Red Discount Plan does not meet the minimum creditable coverage requirements under M.G.L. c. 111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. The list of participating providers is at http://www.qcdofamerica.com/finddentists/index. A written list of participating providers is available upon request. You may cancel within the first 30 days after receipt of membership materials and receive a full refund, less a nominal processing fee (nominal fee for MD residents is $5, AR and TN residents will be refunded processing fee).
